The office of Vice president Sara Duterte is bringing her services closer to Negrosanons with the inauguration of a satellite office in Bacolod City.
The office, located in the Mosser Building in Lacson Extension, is the OVP’s seventh satellite office in the country, and it was formally opened yesteday, August 1, by former mayor Evelio Leonardia and former councilor Ricardo Tan.
The office, which will handle requests for medical and funeral assistance, was made possible by Leonardia, a Duterte supporter.
To be eligible for the assistance program, applicants must submit an application form, a copy of a medical/clinical abstract, a social case study report, or a certificate of indigency from the barangay, and a valid identification card of the patient or his representative.
The OVP specified in a copy of the checklist it published that anyone requesting medical assistance must include a copy of the doctor’s prescription, treatment protocol, and cost estimate for the medications.
Meanwhile, a request for hospital assistance must be accompanied by a statement of account, a referral letter, and a promissory note if the patient has already been released. The checklist added that requests for diagnostic procedures, operations, surgeries, and transplants must be accompanied by a hospital quotation.
Meanwhile, requests for burial must be supported by a copy of the decedent’s death certificate and a valid ID.
