Application for health card & health certificate in Bacolod now online

The City Health Office of Bacolod has launched its first ever online appointment system to address the long queue of individuals lining outside of its office along Galo Street to secure a health card or a health certificate.

Since the pandemic, hundreds of individuals line up as early as dawn outside of the city health office to secure a slot to process their medical certificate or secure a health card. But with the online application, the health department hopes to avoid the queue that sometimes reaches up to Burgos St.

According to CHO, all appointments for both health card and health certificate requests has to be done online effective January 4, 2021. No walk-in applicnats will be entertained according to the health department.

To secure a slot, applicants can log on to https://bacolodcityhealth.com/

Applicants can book as early as now for a slot on January 4. Upon logging on to the site, applicant may select his preferred appointment date and fill-out all the required information to book an appointment.

Once the application is completed, the system will generate a code and the applicant will only need to print the appointment schedule and the code, and present it at the CHO office on his appointment date.

The City Health Office will still require applicants to bring all the original copies of their requirements before a health card or health certificate can be released.

As for those who are scheduled to renew their existing health card, the CHO has scheduled application for renewal starting May 3- December 29, 2021 only. The health department won’t accept any bookings during holidays.
